Disability Living Allowance (DLA) - Forms
Applying for Disability Living Allowance (DLA) and Personal Independence Payment (PIP) can be a challenge if you find it overwhelming for whatever reason, do give us a call – 01278 691100 and we will help. However you may find that with the help of the documents provided here you can do it yourself – the ‘DLA Guidance notes’ are particularly helpful as are the ‘example’ DLA forms. Again – if you have any questions do give us a call.
NB – DLA is split into two components ‘Care’ and Mobility’. Generally speaking the majority of our children DO NOT qualify for the high rate of the mobility component because of any physical difficulties but usually DO qualify on the grounds of “severe mental impairment and challenging behaviour” So it is important NOT to state how far the child can walk but to ignore this question and answer ‘not applicable’ or simply answer ‘no’ to the question “do you have any physical problems that restrict your walking”.
